Wolverhampton Total Health Primary Care Network

Welcome to Wolverhampton Total Health Website which we hope will provide patients registered with our practices useful information on the services we provide.

Wolverhampton Total Health Primary Care Network is a group of Wolverhampton GP practices working together to increase your access to services and appointments.

The aims of the Primary Care Network are to try to maintain the individual GP practices that you know whilst working together as an extended team to share some of the specialist skills that only some practices currently have, and to offer new services. We hope to increase the quality of the care that you receive in this way and create more patient centred care closer to home.

Care Navigation

Our reception teams may ask you for the nature of your problem to establish who best to book you with or to re-direct you.  It is part of a National Programme called Care Navigation.

Care navigation is a person-centred approach that uses signposting and information to help Primary Care patients and their carers move through the health and social care system, as smoothly as possible. It enables frontline staff to provide patients with more information about local health and wellbeing services, both within and outside of primary care, in a safe, effective way.

This is a tried and tested model of care that has been tailored to local needs and services to improve access to Primary Care services for patients and reduce GP pressures all in one.
